3 Intravenous Sedation.

 

 

Except for the simplest of cases most patients/children will have an Intravenous Cannula (needle) inserted into their arm. This is an important safety practice. It is usually inserted while the patient/child is breathing Low Dose Sevoflurane. With the technique used at Chartwell most children will feel very little of the injection.

 

Principle of minimum sedation :- At Chartwell we believe that treatment should be carried out using the smallest dose of sedative possible necessary to achieve co-operation.

 

In spite of this we find that often patients/children referred to Chartwell for treatment, often require some intravenous sedation.

 

The reason for this can be:-

•   Very young children.

•   Very painful work.

•   Anxiety and needle/dental phobia.

•   Extraction in the presence of infection when local anaesthetic is often less effective.

•   Multiple fillings and extraction where treatment is prolonged.

 

Drugs used for intravenous sedation include:

 

Midazolam      =      sedative/amnesic

Fentanyl         =      opiate analgesic/amnesic

Alfentanil        =      opiate analgesic/amnesic

Propofol          =      sedative and I.V anaesthetic agent      

 

Intravenous agents can be used in combination with Nitrous Oxide (laughing or happy gas) and sevoflurane which is a very powerful anaesthetic agent used in low concentrations, to potentiate the effects of intravenous drugs.

 

Different drugs will be used depending on age, the length of treatment, whether one is having oral surgery, orthodontics, fillings, restorative dentistry or dental implants.

 

Chartwell Dental Clinic specialises in the treatment of children under sedation. Children as young as 2 have been treated for emergencies. However as a general rule we prefer that the child is at least 5 years old and over 15 Kilos.
